Hi and welcome to Fort Dorchester Elementary School(FDES). I am excited to return as the nurse of such an amazing school. I have been the school nurse at FDES since our doors opened in 2002. I was born and raised in Charleston, SC and have lived here my entire life. I graduated from The Medical University of South Carolina, College of Nursing and have always worked in Pediatrics. My background includes Behavioral Medicine, Cystic Fibrosis, Orthopedics, and Neurology. I currently live in Summerville with my husband and kids. Between the 2 of us we have 6 children, 5 of whom attend Dorchester School District Two Schools...a 7th grader, 6th grader, 5th grader, 4th grader, and a 3rd grader. Our 17 year old attends South Pointe in Rock Hill. When I'm at work at FDES I consider everyone here family; which includes, approximately 150 employees and 1100+ students. This Dorchester School District Two Medication Permission Request Form has to be completed and signed by a doctor and the parent/legal guardian. The medication to be given has to be brought to the school by a parent/legal guardian in it's prescribed labeled bottle or if over-the-counter, in it's original container. |
